Patient navigation
The patient navigation strategy aims to provide holistic patient care through intensive case management. Patient navigators work one-on-one with clients to encourage continuation engagement and adherence to medical treatment, access to social services, improved communication and rapid re-engagement in care.

Coordination of the healthcare team
Care team coordination allows STEPS staff to establish and evaluate care plans to meet client needs across the continuum of care and improve adherence and retention outcomes. Regular care team meetings ensure that the team remains up-to-date and responsive to the client’s changing needs and circumstances. The care team typically consists of a patient navigator, care coordinator, primary care provider, and other important medical and service providers. The heart of care team coordination is open and consistent communication and collaboration between healthcare providers with the aim of improving customer outcomes.

HIV self-management
Building a strong relationship with your clients is the foundation of HIV self-management, a process by which patient navigators empower their clients to take control of their HIV and manage their overall health. Patient navigators work one-on-one with clients in patient navigation sessions using information and educational tools found on the HIV Self-Management website and workbook. The goal of HIV self-management sessions is to help clients work toward manage their own health and increase adherence to their treatment plan through coaching, advice and accessible content.
